Credentialing Process

Once you’ve toured the facility and confirmed interest in joining our surgery center, you may begin the privileging process.

To request privileges, you will need to:

Complete the Privileging Application
Provide required documentation (license, DEA, malpractice, CV, certifications, immunization records, etc.)
Submit your packet to the Administrator
Await committee review and approval
